Resumen

Objetivo: Explorar los factores asociados al uso del antibiótico implicado en una reacción adversa, posterior a una prueba de provocación negativa, en pacientes pediátricos que acuden al servicio de Alergología clínica de una institución de salud en Medellín.

Métodos: Estudio observacional, de corte transversal, con intención analítica, en pacientes pediátricos, con antecedente de reacción adversa a un antibiótico, a quienes se les realizó una prueba de provocación y obtuvieron un resultado negativo. Los datos sociodemográficos y clínicos se obtuvieron de las historias clínicas y se efectuó una encuesta a los padres o cuidadores de los pacientes que cumplieron los criterios de inclusión.

Resultados: Entre enero de 2016 a diciembre de 2021 se realizaron 571 pruebas de provocación. Sólo el 19.2% de los pacientes recibió el antibiótico implicado o de la misma clase después de una prueba de provocación negativa, sin tener reacciones adversas relacionada con el nuevo consumo, y el 53% no recibieron el antibiótico, a pesar de necesitarlo, principalmente porque sus padres-cuidadores continúan reportando el rótulo de alergia. Adicionalmente, el 26.9% de los padres-cuidadores no recordaban el resultado negativo de la prueba, y un 44.2% rechazarían la administración futura del antibiótico en los niños, identificando el no recordar el resultado negativo de la prueba de provocación como un factor asociado a esta conducta.

Conclusión: Deben diseñarse estrategias locales (entrega de formatos individualizados para padres y médicos) que expliquen el significado del resultado negativo de la prueba, y el seguimiento en la consulta verificando el apego a las recomendaciones y de esta forma aumentar la efectividad del desetiquetado de alergia a antibióticos.
